Switching between connections should clear the form state completely

      the form state is not completely cleaned up when selecting a connection. The immediate issue is that the connection error doesn't clean up, but in general that may lead to more serious issues.

      We want to ensure the entire state of the form is completely clean when a connection is selected from the sidebar.
