Remove deprecated API elements

    • Major Change

      There are currently 347 API elements that are now deprecated.  As part of the 4.0 release all of them should be removed, except for those clearly marked as not intended for removal.  There are only two of those:

      • com.mongodb.Mongo#getDB
      • org.bson.json.JsonMode#STRICT

      though that could change based on user feedback.
