2.0.42 -> 2.0.43 broke deploy "MongoError: not master"

      Tested with iojs v2 and nodejs v4

      After updating to 2.0.43 my deployment with a replicated mongodb started failing with the message "MongoError: not master".
      It seems to happen when "ensureIndex" is called. I'm not sure whether other commands would run or not.
