Clear connection pools when monitor ismaster times out

      When the ismaster on monitoring connection times out (after any retries), the driver should clear connection pools for servers being marked unknown.

      The new behavior is, any time a server transitions from non-unknown to unknown state the pool is being cleared, except if the transition is a result of not master-type error and the server is 4.2+.
