Replace all usage of heartbeat op times with lastAppliedOpTime / lastDurableOptime

      We only need one notion of last applied op time and last durable op time in the replication coordinator and topology coordinator; this should be the one in the memberHeartbeatData, which is the latest of heartbeat and replSetUpdatePosition times.
