Objectives:

1. rationalize update code with a clean API for manipulating docs
2. compatibility: continue to support existing structures, (e.g.) fast in-place updates
3. provide a framework for efficient aggregation pipeline implementation
4. support document fragments, creating nodes and heap entries as needed
