index corruption when inserting documents, mongod now crashes on startup

      kernel version: 2.6.32-45-server

      Fairly routine insertion of a new document as far as I can tell. Was in the middle of inserting a few 1000 documents and mongod crashed with a stack trace (attached). Now when I start mongod, I get the same stack trace on startup.

      Appears to be complaining about btree corruption, but I cannot even start the server in order to reindex or repair anything.
