Force CollectionCatalog::lookupCollectionByUUID to be always called with a db name

      Given that UUID uniqueness is only enforced on a per-database bases and given that the authorization system is db-based, all lookups by collection uuid done on the local catalog must be switched to lookup by database name AND uuid.
