Review how LSM determines whether chunk cursors are positioned

      LSM needs to track whether chunk cursors are positioned. It currently looks at the WT_CURSTD_KEY_INT flag to decide that, which means LSM is making assumptions about how file cursors will behave. Find a better solution.
