Schema format 's' with no size, using an index, fails during insert

      In the attached program, the main table has value format 's'. An index is created that uses the value from the main table as its key. Insert() of a k/v pair returns ENOMEM.

      The problem does not occur with no index.

      The problem does not occur with format '1s'.

      Possibly related to WT-1998.
